Congratulations! Your child has qualified to participate in the 4th/5th Grade Talent Search sponsored by the Duke University Talent Identification Program (Duke TIP). The Duke TIP 4th/5th Grade Talent Search is aimed at students who possess high academic ability by having scored in the 95th percentile or above in portions of the ITBS (Iowa Test of Basic Skills).

Please take time to sit down and read the brochure that accompanies this notification form for details about the benefits of the Duke TIP 4th/5th Grade Talent Search as well as how to register. There is a fee of $35 to participate. You may enroll using one of two methods: online Web registration or paper application. On-line registration may be completed at the Duke TIP website: www.tip.duke.edu/45enroll. If you choose to register with a paper application, a computer generated application (bubble form) is attached and should be completed and returned to Duke TIP. This notification form includes some important information that you will need in order to complete the application if you decide to enroll. You may contact Faith Shiver, your schools contact for the 4th/5th Grade Talent Search at (229) 225 - 2625, or contact the Duke TIP office directly at (919) 668-9100 for further assistance.

Registrants must complete all sections of the 4th/5th Grade Talent Search application including:
