State Health Benefits Plan (SHBP): Please take a moment to carefully review the Employee Decision Guide, as it has been created especially for you to help make an informed decision. After you carefully review the Decision Guide, follow the enrollment instructions through our online web portal www.mySHBPga.adp.com and choice the coverage option you believe offers the best choice for you and your family.
There are two forms: Enrollment Form or Active Employee Declination Health Coverage form. You must return the form that applies to you. If you are covering your spouse or children, State Health will mail a letter to you asking for a marriage license and/or birth certificate for each child. You must return the letter from State Health, marriage license and/or birth certificates to SHBP. You can scan and upload or fax the information to State Health, If you do not return the letter along with the marriage license and/or birth certificates they will drop your dependents. Please make sure that you open everything that comes in the mail from State Health. If you have any questions contact Becky Donathan 225.2666 ext. 1016. This also applies the Qualifying Events. |
Employees Retirement System (ERS)/Public School Employees Retirement System (PSERS): It is very important that you go to ERS website and register. Click on the ERS/PSERS tab. Once you've registered you must add your beneficiaries. To see the ERS/PSERS Handbook look on the left hand side of the page, click Members Education, click on ERS/PSERS Handbook. You can download or print the file.
Steps to register: click on Account Access, then Account Access again and register and follow the instruction. Once you've registered you must add your beneficiaries. Teacher Retirement System of Georgia (TRS): It is very important that you go to the TRS website and register. Click on the TRS tab. See steps to register below. Once you've registered you must add your beneficiaries. TRS Member's Guide: click on the TRS Website tab, scroll to the bottom of the page, click on publications, scroll down to Miscellaneous, click on Member's Guide. You can either download or print the file. Steps to register: click on the TRS Website Tab, click on Active Member, on the tool bar click on Account Login, click on Active Member, then register. |
MassMutual is mandatory for all employees. Thomasville City Schools do not take out social security. The 403B replaces this. The system contributes 2% and your part is 4%. There are two forms one is mandatory and the other is voluntary. Group number 750255 is mandatory and Group number 750254 is voluntary. You must return the mandatory form to Becky Donathan.

Big Oaks Benefits: You will enroll in all your benefits here except State Health. Click on the Big Oaks Benefits tab. Click on New User Registration, Company Identifier: tcityschools. To begin your enrollment you will need to create your account. Once you have created your account click "Start Benefits" to begin your elections.

AFLAC Supplement Plans: Thomasville City Schools offer employees the option of payroll deduction as a convenient way to pay your Accident, Cancer or Critical Illness Plan. It helps you cover your medical expenses by paying tax-free money, directly to you.
The Accident Plan pays you a cash benefit if you are injured due to an accident. The Cancer plan pays you an initial lump sum benefit when cancer is diagnosed, for each procedure. The Critical Illness plan pays you a cash benefit if you incur a condition such as cancer, coma, stroke, paralysis, heart attack, end stage renal failure and more. Transamerica Universal Life: Thomasville City Schools offer Transamerica Universal Life plans, with guaranteed issue of up to $150,000 for employees. UNUM Long Term Care: This plan pays you a monthly benefit during a time in which you are unable to perform "activities of daily living" and requires assistance.
